Management code area is limited to 5 characters. Indicate a range or specific codes to print in the Management codes/range box above, or indicate if do not want any codes printed on the tags. Theline of text is limited to 20 characters.

A USDA Premises ID is needed for ordering USDA tags.  A Premises ID is seven digits long, usually starting with "00" followed by five alphanumeric characters.  If you cannot find or do not have a Premises ID, contact the USDA or your state’s Animal Health Commission.

Premises ID is required for all USDA tag orders.
