USDA Maxi tag, size 4" high x 3" wide, button back is included. Management code of your choosing and EID number (same as EID number on EID tag) printed on front. 4 lines of text on back. Half duplex EID tag with mgmt code printed on EID button back.

Management code area is limited to 5 characters. Indicate a range or specific codes to print in the Management codes/range box above, or indicate if do not want any codes printed on the tags.

The management code is the largest number/code on the tag, such as a tag number. This is laser-engraved with an ink overlay for long-term readability.

A USDA Premises ID is needed for ordering USDA tags.  A Premises ID is seven digits long, usually starting with "00" followed by five alphanumeric characters.  If you cannot find or do not have a Premises ID, contact the USDA or your state’s Animal Health Commission.

Premises ID is required for all USDA tag orders.

New logo: Upload a black & white digital image. The image must be a png or jpg file. Images cannot be hand-drawn. A proof will be sent via email before it's printed on the tags.
